What are Homebound Services:
Homebound Services provide instructional support for students when medical circumstances prevent regular school attendance. Homebound instruction consists of two 45-minute sessions per week, or two 60-minute sessions per week for students with special accommodations. The homebound teacher will coordinate with your student’s classroom teacher(s) to collect coursework and resources to ensure continuity of instruction. During the two weekly sessions, the homebound teacher provides one-on-one instructional support and communicates progress updates to the classroom teacher to help your student stay on track academically to the best of their ability.
Who is Eligible:
To qualify for homebound services, a student must:
- Be enrolled in Northwest Community Schools
- Have a medical diagnosis from a licensed physician (M.D., D.O., or certified physician assistant) that prevents full-time school attendance.
- Be expected to miss five or more consecutive school days due to the medical condition.
How to Enroll Your Student:
- Consult your physician regarding whether homebound or a reduced school day are needed for your student.
- If services are necessary, have your physician [Must be an M.D., D.O., or Certified Physician’s Assistant] complete the form linked HERE Have the completed form sent to Erin Darnell via email at erin.darnell@nwschools.org or by delivering the form directly to your school building’s front office.
- The district's homebound provider will reach out to you using the contact information on file with the district to arrange services.
- Services will be provided twice a week, for the length of time indicated in your student’s physician’s statement. Any changes to Homebound services will require an updated note from your student’s doctor.
How Homebound Services Work:
Once a student qualifies for homebound services, a district representative will reach out to you to discuss how the district can best support your student and family in receiving services.
Services are provided in the format that best supports your student. Instruction may be delivered in-person or virtually. If virtual services are the best option and your student does not have access to a computer, one can be provided. In-person services can take place at any location that best facilitates your student’s learning, including a library, the hospital (if your student is an inpatient and their medical team approves), another public location, or your home.
