Required MHSSA Physical
ALL athletes for the 2025/2026 school year MUST have an updated MHSAA Physical (dated after April 15th, 2025) on file in the HS Athletic Office – NO EXCEPTIONS!
Along with the MHSAA Physical ALL athletes MUST have the Athlete/Parent Responsibility Form and the Henry Ford Allegiance Consent for Treatment Form – NO EXCEPTIONS!
Please make sure they are COMPLETELY filled out in EVERY section and SIGNED in EVERY section. If they are not completely filled out/signed and/or an athlete is missing a form, they will NOT be able to participate/try-out until everything is turned in.
